The purpose of the Bristol HEZ Community Wellness Survey is to hear from community residents about how the Bristol Health Equity Zone can support wellness activities in Bristol. The survey is completely voluntary. However, you have the option of providing your contact information at the end of this survey to enter into a random drawing for one of three $35.00 gift cards to local restaurants. To be eligible to win, you must complete the survey by January 31st.
